Oliver Hazard Perry sınıfı fırkateyn, ABD Deniz Kuvvetleri'nde hizmet etmiş olan fırkateyn sınıfı. Adı 1812 Savaşı'nda ABD Deniz Kuvvetleri'nin zaferine katkıda bulunup "Hero of Lake Erie" ("Erie Gölü Kahramanı") olarak anılan Komodor Oliver Hazard Perry (Matthew Calbraith Perry'nin ağabeyi)’den alınmıştır.
FFG-12 adını II. Dünya Savaşı'nda ölen komutan George Philip’ten aldı. FFG-12 1980 yılında denize indirildi, 2003 yılında hizmetten alındı. FFG-14 ise adını Amiral John H. Sides’tan aldı. 1981 yılında hizmete giren FFG-14 İran-Irak savaşı sırasında İran Hava Yollarına ait IR-655 sefer sayılı yolcu uçağının vurulması 60’ı çocuk 290 yolcunun ölmesi olayına dolaylı olarak karıştı. Sides, bu sırada İran karasularındaydı. ABD hükümeti İran’a 61,8 milyon dolar tazminat ödedi.

- Amerika Birleşik Devletleri: 1977 ile 1989 yılları arasında ABD Deniz Kuvvetleri'nde 51 fırkateyn kullanılmıştır. 29 Eylül 2015 tarihinde bu sınıfa ait son aktif fırkateyn hizmet dışına çıkarılmıştır.
- Avustralya (Adelaide sınıfı): Avustralya Kraliyet Donanması 6 fırkateyni satın almıştır. Bunların 4'ü ABD'de ve diğer ikisi Avustularya'da inşa edilmiştir. 2000'li yılların başında 8-cell Mk.41 VLS ve 32 Evolved Sea Sparrow (ESSM) füzeleri ile güncelleştirilmiştir. 2016 itibarıyla 3 fırkateyn aktif hizmettedir.
- Bahreyn: 1996'da ABD hükûmeti tarafından USS Jack Williams (FFG-24) hediye edilerek Sabha (90) olmuştur.
- Mısır (Mubarak sınıfı): 4 fırkateyn ABD Deniz Kuvvetleri'nden devredilmiştir.
- Polonya: 2002 ve 2003 yıllarda 2 fırkateyn ABD Deniz Kuvvetleri'nden devredilmiştir.
- Tayvan (Cheng Kung sınıfı): Tayvan yapımı 8 fırkateyn Harpoon yerine Hsuing Feng II anti-gemi füze ile donatılarak inşa edilmiştir.
- İspanya (Santa Maria sınıfı): İspanya yapımı 6 fırkateyn.
- Türkiye (Gabya sınıfı): ABD Deniz Kuvvetleri'nden 8 fırkateyn devredilmiştir.
